Change wording of answers in the Self-Certify popup
The default wording in the self-certify popup ‘I confirm that I have read and agree to all statements above’ and ‘I do not agree to all statements above’ do not match how we want to word the statements in the popup.

Checkboxes [x] are used instead of options (*) - that's confusing, because we can't select both, so option fields should be used.

The other issue is that these are two check boxes that users can select both, of a question you only need one answer to.
Better that this is a radio button to select one or the other.
